- Hypertensive encephalopathy: Severe headache, altered mental status, visual disturbances. Acute heart failure: Pulmonary edema, shortness of breath. Acute coronary syndrome (ACS): Unstable angina or myocardial infarction with elevated blood pressure. Aortic dissection: Severe chest pain radiating to the back, unequal blood pressures in arms.
Remember: IV push metoprolol is Not first-line treatment for all hypertension. Oral medications are generally preferred unless immediate hemodynamic stabilization is required.
Dosage and Administration: Administer slowly, usually at 5mg over 2-5 minutes. Closely monitor blood pressure and heart rate during and after administration. Adjust dosage based on patient response and clinical picture. Frequent vital signs monitoring is absolutely necessary. Continuous ECG monitoring is highly recommended.
Contraindications: Avoid in patients with significant bradycardia, heart block, cardiogenic shock, or severe asthma/bronchospasm. Use caution in patients with peripheral vascular disease or diabetes.
Adverse effects: Watch for hypotension, bradycardia, heart block, and bronchospasm. Have appropriate resuscitation equipment readily available.
